Participation Planning Forms
No, we don’t promote the creation of more paperwork! But we do believe that some forms can actually save time rather than just take it up. Using a form for participation planning provides a structure for the planning process, a clear visual for people who are new to the team or would like a quick refresher of the plan, and a vehicle for communicating the plan between school and home. The Transition Team has an assortment of participation planning forms available online that may 1) work for you as is, or 2) serve as a catalyst for creating your own version. Count the cost!
